The Beacon Community Program vision encompasses advancing patient-centered care through electronic health record (EHR), the outcome of the program is for better health and better care at lower cost.

Local health IT resources used to bind doctors, hospitals, and community and federal programs together for better quality
Authorized under the American Recovery and Reinvestment Act (ARRA) of 2019
Built upon care management, collaboration, and connected electronic health records

The fifteen communities are located throughout the United States
Vary in population and demographic characteristics
High racial and ethnic minorities included
Participating communities have far more advanced EHR adoption rates and each community has different strengths that will help improve efficiency and population health (Beacon Communities Aim to Use Health Information Technology. (n.d).

Improving The Quality & Access of Healthcare Through Healthcare IT

Reduction of preventable hospital readmissions for high-risk/high-cost populations
Improving patient and provider preparation for the transitional period post-discharge
Promoting appropriate emergency department utilization for high-risk/high-cost populations
Improving chronic disease management
Seamless, cost-efficient transitions for patients moving between healthcare settings
Deploying technology-supported staff focused on care coordination
Developing a state-wide health IT infrastructure that allows healthcare professionals to share data securely and appropriately, and serves as the prototype for advanced state-wide care management systems

Status Update
Baseline data collection has been completed in Stanly and Cabarrus Health Departments and is about 50% complete in the Rowan County Health Department.
The North Carolina Public Health Portal compiles data from Medicaid, Medicare, the U.S. Census, Institute of Medicine, and other indicators in a geo-mapping tool.
Data is representable at the county, census tract, Community Care Network, and practice levels.
Anna, the virtual home monitoring pilot has enrolled nearly 100 patients with several success stories.

Can Replication of Beacon Community Be Achieved Locally?
It is my find that Southern Piedmont Beacon Community can be replicated in my community of Tampa, FL. This conclusion has reached an understanding that Tampa’s local health community and all its key healthcare facilities already participate in EHR’s. With an extensive teaching base and significant partnerships between the state, county, HCO’s, and learning institutions, it would be relatively easy and beneficial for this replication.
