Chief complaint: Patient states I havent been feeling too well lately. My knees are killing me. So is my back. I took my blood pressure at the supermarket and it was, 165/95, which sacred me a little bit.
He gained some weight since his last visit. He were 156 lbs and gained 38 lbs. What is too much to carry in his 5.6 frame.
Labs tests results showed that his cholesterol and fasting blood sugar were elevated
He use to work out on the dock, which was very physical. He thought it was killing him, so he put in for the manager position. He works with a lot of paperwork which it is insane for him and he never leave from his desk. He feels so stressed all the time. Before he would clock in and clock out. Now he works 12 t0 14 hours days. He having been grabbing a lot of fast food on the way home because his family has already eaten dinner. After he eats, he feels so bloated. He used to go to the gym every day, after work at four, but now he works too much and cannot make it anymore. He havent been to the gym in probably eight months.
