Community Health Assessment
Jordan, Minnesota is a rural community with a population of 6,500. It is composed of various schools, churches, stores, parks, and hospital care. There are neighborhoods located downtown Jordan as well as communities just outside the main city center. It has low and middle income families are various levels. Major roadways connect Jordan to more populated parts of Minnesota.
Executive Summary
General Condition and Public Health Needs
The general conditions of Jordan, MN is that is has a stagnant growth rate. While it promotes itself for expansion, the council want to keep its small town charm. It has adequate schools and health care facilities for the size of its population. St. Francis Health services support 2 major health providers and they also have a senior living facility. It does seem that as Jordan expands or new business and industries move in, the need for better public health services will be necessary. There is a grocery store that has been updated and a shopping plaza that does have several vacancies. Additionally, neighborhoods that are located downtown tend to have lower socio-economic characteristics. Many rural communities are becoming appealing to the general market. When this trend increases, the likelihood of cities like Jordan can expect new growth potential which would impact the need for more public services and medical care facilities.
Environmental Analysis
There are several environmental factors that are at play for the city of Jordan. First, there appears to be limited growth currently. Without a community demand for more shopping, schools, hospitals and supporting infrastructure, there is a chance that this community will have a flat growth rate. Secondly, what are the anticipated growth in the next 4 to 10 years? This information could significantly impact decisions the city and the community will make now.
The social determinants of health tend to be in relation to activities residents are provided within the city. For example, Jordan has a 16-acre Lagoon Park that has walking paths and exercise options for its residents. Studies show that communities with these types of features tend to promote better health options which can be have long-lasting positive benefits for the society. This would mean less visits to their doctors and fewer hospitalizations. With this said, there is a lower income population that lives downtown. This could create a higher demand for doctor offices and more frequent hospital visits (typically due to lack of exercise, poor diet, limited financial resources).
Conclusions
In order to obtain a quick overview to understand its population and its primary health concerns, a useful tool is the windshield survey. This is a systematic observation to access and comprehend the community holistically. With the windshield survey, the nurse leader can simultaneously construct the community map as a way to easily understand and illustrate the community infrastructure and potential healthcare needs (Daemmrich, A. A. (2016). In Jordan, MN, this small, rural town has basic necessities such as shopping, schools, medical facility, parks and housing. The downtown area is more condensed, however, is low income neighborhood. Just outside downtown, near the schools, are the neighborhoods which are more affluent. This can create a health care dynamic which means that health care facilities (doctors, hospitals) would be largely used by the lower demographic group (education, diet, financial resources).

Overview
Draft a 35-page change proposal to executive leaders, soliciting their support for a change to the health care
system in the community you selected in the previous assessment.
Note: This assessment builds upon the work you completed in the previous assessment. Therefore, complete the
assessments in the order in which they are presented.
To improve health care outcomes within a community, leaders must understand the needs of specific
populations. In addition, it is important that leaders are able to gather and present persuasive evidence to
decision makers to obtain the necessary funding and establish these needs as priorities for budget and resource
planning. Thus, proposed changes should support improved outcomes and align with the current or needed
health care policy.
This assessment provides an opportunity to propose a community health care change that is based on the
community health assessment you completed in Assessment 2. By successfully completing this assessment, you
will demonstrate your proficiency in the following course competencies and assessment criteria:
